State licensing boards:
• NPI : National Provider Identifier
• OIG : Office of Inspector General
• SAM : System for Award Management
• Medicare
• NPDB :National Practitioner Data Bank
Hospital privileges, practitioner education and work history. Physicians self-report non * data and attest to its accuracy on CAQH every 6 months.
Hospitals:
• Healthcare Facilities Accreditation (HFAP)
• CARF (Commission on Accreditation of Rehabilitation Facilities) accreditation
• OIG : Office of Inspector General
• SAM : System for Award Management
• Medicare
